Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services.

• You have the right to receive a “Good Faith Estimate” explaining how much your therapy services will cost.
• My fee is $150 per 50-minute session.
• The total cost of services depends on the number of sessions you choose to attend.
• You will receive a written Good Faith Estimate before your first session, and you may request one at any time.

Record Request to Find Rest Christian Counseling

To request your counseling records under Texas law, please submit a written, signed request that includes your full name, date of birth, contact information, and a clear description of the records you want. If you want the records sent to a third party, include the name and address of the recipient, the purpose of the release, and the dates of service requested. Under the Texas Medical Records Privacy Act (Texas Health & Safety Code §181) and the confidential mental health records requirements in Texas Health & Safety Code Chapter 611, clients have the right to access their records, and written authorization is required for disclosures to third parties. If your client portal was previously closed after the completion of services, please email our office at counselor@findrestcc.com to request temporary reactivation. Once the portal is reopened, the system will automatically send an email prompting you to sign in, which serves as an additional layer of identity verification. All record requests and forms, including upload of a government-issued ID, must be submitted through the secure client portal for your confidentiality. Please do not send sensitive documents by regular email. Processing times may vary, and Texas law permits providers to charge a reasonable, cost-based fee for copying and delivering records. Notice of Consumer Complaint Process with the Texas Office of the Attorney General

If you have a consumer complaint regarding business practices, deceptive acts, or unfair practices, you may file a complaint with the Texas Office of the Attorney General – Consumer Protection Division. The Office of the Attorney General accepts and reviews consumer complaints to enforce consumer protection laws in Texas.
